1244 Wound Dehiscence and Infection of a Total Knee Replacement Secondary to Postoperative ‘Dorsal Fin’ Intra-Articular Vertical Patella Dislocation

Abstract A 75-year-old man presented to our emergency department with a bleeding wound 19 days following primary hinged left total knee replacement, performed elsewhere, for lateral tibiofemoral osteoarthritis. Examination revealed held in fixed flexion of 30°, partial dehiscence the wound, implant exposure and visible patella. Radiographs an abnormally raised joint line vertical appearance patella on image. The decision was made proceed two-stage revision. At operation, confirmed be vertically positioned trochlea groove femoral component identified as likely cause dehiscence. There evidence large release having been during surgery. orthoplastic debridement performed, together removal, static cement spacer insertion. Following eight-week interval IV antibiotics, second stage performed. new implanted, metaphyseal sleeves stems. tibial tubercle osteotomy required address tracking gastrocnemius flap necessary distal soft tissue defect. This case study highlights unusual arthroplasty. Patella dislocation axis rotation has described native knee1, but knowledge, this is first report phenomenon almost certainly secondary malpositioning emphasizes significance restoring correct replacement
